Compensation & Benefits 

• $115,000+ (Straight salary – negotiable) 
• $10,000 Sign-on bonus (negotiable) 
• Relocation assistance  
• $1,200 CE stipend + 5 CE days 
• 3 weeks PTO year one, 4 weeks year two & beyond (additionally 6 paid holidays/year)  
• Dues, license fees, and practice liability insurance covered  
• Medical and EAP insurance provided by employer with partial premium coverage 
• Dental & Vision Insurance 
• Short-term & long-term disability and life insurance offered through AFLAC 
• 401k with employer match up to 4.5%  
• Uniform allowance
